Grant/Check Requests

Grant requests must be signed by two booster club executive board members named on the clubs "Adoption of Bylaws". Additionally, all necessary documentation and receipts must accompany the request in order to be processed. All requests should be emailed to  madisonfoundationtreasurer@gmail.com. Allow 7-10 business days for processing and receipt.

Fundraising Requests

All fundraising requests require Foundation and School Administration approval prior to conducting any fundraisers. Please complete the Fundraising Request Form and email to foundationmadisonhs@gmail.com. Fundraising requests must be submitted at least two weeks prior to fundraising activity and can not conflict with another organization conducting the same fundraiser.
